Client Profile:

Due to confidentiality, we will refer to our client as Florence, however, this is not her real name.

Background:

Florence comes from small family of one sister and one brother. She has lived all her life in the city. She has two children, a son and a daughter, the daughter lives abroad and her son lives an hour away. Her son and his two children visit Florence every Sunday. Florence speaks with her daughter on the phone regularly and she visits her mom bi-annually. Florence husband passed away some time back and she has been living independently.

Medical:

Up until Florence’s 71st birthday she was active and enjoyed gardening.  Florence has diabetes and has been finding it difficult to manage it in recent times due to the onset of dementia. As a result of this Florence has moved to Cherrywood Nursing home which caters for 64 residents. Florence has had a fall recently and is currently using a walking aid to move about more independently.

Florence has blurred vision as a result of her diabetes and now requires assistance with various aspects of daily living

She is prone to skin infections and she heals slowly from cuts and bruises.

Florence is also feeling very low due to her lifestyle change

Conclusion:

• Florence is missing her independent life.
• She requires additional assistance especially since having her fall.
• Her eyesight is deteriorating.
• She is missing her gardening.
