Assessment Brief

To complete this assessment, you must create a PowerPoint presentation or Word
Document essay. In this presentation you will construct three diverse profiles that
demonstrate how different social determinants of health might intersect to create a
unique picture of three individuals’ health and wellbeing. These profiles must be
entirely fictional – in that they may share characteristics of real people, but no
individual profile can be based on the identity of any real person. This requires that
you think about the different social determinant of health likely to impact different
groups, at different times, in different locations and use this information to construct
three fictional profiles.

Once you have identified the three profiles you need to identify three different
resources, services or activities you might refer them to in order of importance. In
identifying these resources/ services / activities you must also outline possible
barriers to participation.

You are required to support your answer with reference to theory e. g. Maslow’s
Hierarchy of Need, and policy e. g. HSE Social Prescribing Framework. In completing
this assessment, you are required to use only desk-based research i.e. anything that
is publicly available: books, texts, articles, online videos, documentaries, newspapers,
reputable websites, academic papers etc. For reasons of ethics and GDPR you
cannot use personal, family or work information, nor can you contact services or
service users.

You are encouraged to envisage these individuals as service users within a particular
context e. g. prison, secondary school, family resource centre, men’s shed etc. This
will allow you to develop a more three-dimensional sense of the relationship between
the social context, community and the social determinants of heath, and how they
intersect to create each individual’s profile.

Assessment Mode:

1. PowerPoint Presentations (using image and text) no more than ten
slides and 800 words.
2. Word Document no more than 1200-1800 words – you can also
illustrate your text with images, but this is not required. Do not submit as a
PDF. Marks will be deducted for going over or under the wordcount
