Comprehensive Health History taking is a core competency of the advanced nursing role. The goal of obtaining a health history is to gain a better understanding and knowledge about the patient’s condition which gives way to establishing a therapeutic relationship between the nurse and the patient.

One way of establishing rapport with the patient is through effective communication. Effective communication between nurses and patients is essential for the successful outcome of individualized nursing care for each patient.

Even before the history taking started, I made sure that the patient feels respected by creating a warm and comforting atmosphere where privacy was observed. Patients are more likely to disclose information when trust has been established.

We, as nurses deal with patients closely, therefore it is extremely important that trust is built as this allows better care experience and satisfactory patient outcomes.
