You are required to keep a journal over a period of time relevant to caring for a person at end of life stage. ft should be a reflective account of working in a Palliative Care setting. As part of this journal, you should include the following:

1 Reflect on a range of communication strategies and processes which are central to the work in Palliative Care

2 Reflect on how you applied a person-centered approach in caring for a person in the last days of their lives. You should include and show the provision of supportive holistic care- including safety, dignity, respect, and comfort.

3 You should reflect on the care needed to respond to their physical. psychological, emotional, and spiritual requirements.

4 Include a record of how you worked effectively and with great sensitivity in relation to the dying person and their significant others in a Palliative Care setting

5 Record and reflect on the range of skills, required from a Health Care Assistant,  to care for a patient at end of life.
