Observational Report Task
You are required to conduct a residential / client observation. For the purposes of this task, you are required to observe the resident and write an observational report using a topic from the list below:
- Social isolation/withdrawal
- Emotional upset
- Communication difficulties
- Memory loss
- Physical disability
Observe the client when he/she is conducting an activity or involved in a situation that poses difficulties for him/her. Record your findings objectively under the following headings:
1. Client Profile – Provide detailed background information, e.g., age, living in a care home/independently, mobility, illness/condition, family support or alone, medication, etc.
2. Provide a detailed account of the challenges experienced by the client in relation to the chosen category above.

3. Note the areas in which he/she demonstrated competence (what they can do well) and confidence.
4. Describe the measures/interventions that are presently being employed to meet the client’s needs.
5. Provide realistic suggestions to improve the quality of life of the client by:
- Making suggestions of actions or interventions the MDT could make.
- Explaining what contribution you can make as an individual to the client’s wellbeing and quality of life.
6. In carrying out this assessment, outline your learning from a personal and professional perspective.
7. Demonstrate evidence of critical thinking, additional research, and correct referencing.
- Ensure the project is well-formatted and follows a clear layout.
