Scenario

You are working in a community setting that provides support to individuals and/or families that may be at risk of social isolation and exclusion (e.g. people with disabilities, older persons, people with addictions, families, or children). The team leader has asked you to develop a set of training materials that could be used as teaching tools for new staff to develop their understanding of effective case management, including assessment, eligibility, and care support planning. They have asked that you produce a presentation and report to accompany the training.

The electronic presentation will critically evaluate the benefits to the individual, professional, and organization of effective case management (D1). It must be produced using appropriate software. Develop extended notes to support the presentation with referenced sources.

The report will provide a critical evaluation of the benefits to the individual, professionals, and organization of effective care planning (D1).

Activity 1

Your presentation with accompanying notes and audio will critically evaluate the benefits to the individual, professionals, and organizations of effective case management.

It will:

• Outline the meaning of case management.
• Explain the importance of effective case management (P1).
• Discuss the key features of case management, including assessment and eligibility (P2).
• Evaluate the complexities of working in partnership to ensure effective case management (M1).
• Critically evaluate the benefits to the individual, professional, and organization of effective case management (D1).

Activity 2

Develop a detailed report that critically evaluates the benefits to the individual, professionals, and organization of effective care planning.

The report should also:

• Discuss how different needs can be met through a care plan (P3).
• It will investigate the review process and the need for a flexible approach to care planning (P4).
• The report should also evaluate how care plans achieve agreed service user outcomes using professional, local, and national standards of practice (M2).
• Critically evaluate the benefits to the individual, professionals, and organization of effective care planning (D1).

You are encouraged to attach sample care plans or leaflets as support documentation.
