Planning:

• Assessment of client / clients’ needs in relation to their activities of daily living, with reference made to clients care plan
• Level of dependence / independence and its impact on client and HCW consultation with the client and respect for the client decisions, outlining the importance of autonomy and involvement

• Good observation skills with regard to the needs of the client during activity

• How clients’ dignity was maintained during activity
• Outline how you reported on activity and how client confidentiality was maintained

Evaluation & Reflection:

In this section discuss the following:

• Seek feedback from the client on the activity
• What was positive and negative from the clients’ perspective?
• What went well and what did not go well from the writers’ perspective, how does activity impact on Activities of living and clients’ needs
