For this assignment, you will research and conduct a needs assessment of a person with a life-limiting illness in the last days of their life. You will then use this information to develop a plan of care that reflects a person-centred approach to care and meets the person’s physical, emotional, spiritual, and social needs using Roper Logan and Tierney’s model of care.
You will then reflect on the learning achieved and what you will take forward into your career in palliative care.

The Assignment will include an Introduction, main body of work and a reflective conclusion using Gibbs or Burton’s Reflective Cycle/Framework and should be written in a professional style showing dignity and respect for the patient. Ensure all sources are correctly cited in your work and a complete reference list is provided at the end of your document.

Introduction

Choose a life-limiting illness to research and base your assignment on. This can be based on a friend, family member, or someone you met through your work placement. Please use pseudonyms if it is a family member or patient for confidentiality.

Main Body

Conduct a needs assessment of a person with this illness in the last days of their life. You may include evidence based on your experience of caring for someone with this illness in the past, experience based on work placement, or from interviewing a person’s family member. You should also consider the person’s previous medical history and current symptoms.

Conclusion

On completion of the Care plan, you should reflect on your own learning and development throughout the process of completing the needs assessment and the care plan. Discussing previous knowledge around end-of-life care and what they will take forward into the workplace when supporting an individual in the last days of life.

Apply a person-centred approach to caring for a person in the last days of their life to include the provision of supportive and holistic care promoting safety, dignity, respect and comfort.
