Assessment of Group’s Needs: In this section you must,

• Present an overview of the group’s health needs
• rationale for chosen health promotion activity, supported by relevant research

Creation of the Activity: In this section you discuss how you planned the activity, with key elements highlighted. Address how you communicated ideas with relevant stakeholders. Outline any ethical issues that needed to be taken account of. SMART goals outlined clearly, and Health Promotion poster designed and presented. Support section with relevant research.

Delivery: In this section you discuss how you delivered the programme /activity. How you
encouraged participation and supported group. Communication and collaboration with all
stakeholders outlined. Support section with relevant research.

Evaluation: In this section you must, critique the overall impact of the activity for the group,
their participation, feedback, challenges, and outcome.

PART B:

Complete the CPD reflection form provided. Reflect on these practical experiences you’re
learning and how you can improve.
