Reintegration is a principal aspect in the management of obstetric fistula, it refers to the process of restoring fistula patients: Public Health Dissertation, UOL, Ireland

University University of Limerick (UOL)
Subject Public Health

Reintegration is a principal aspect in the management of obstetric fistula, it refers to the process of restoring fistula patients into their environment or a new environment following their experiences of incontinence of urine, faces and in the worst situation which leaves them in shame, loss of dignity and stigma. This is an intervention aimed to ensure that women regain their healthy and productive lives.

On the other side, Social rehabilitation in obstetric fistula is defined as sets of experiences that strive to improve a patient’s quality of life, and it is aimed at managing complications that may arise from this condition. The complications associated with obstetric fistula are beyond the physical effects, it includes the social, mental, and psychological well-being of the patients whose consequences are severe, and thus, women are often ostracized from their community, family, and support groups. In obstetric fistula, the term Social reintegration and rehabilitation is used interchangeably as their both share the same goal.

The prevalence of obstetric fistula is highest globally in Sub-Saharan Africa and Asia with Nigeria having 40% of the global burden in terms of mortality and morbidity. Although efforts have been placed to eradicate this public health problem and its sequels, every year, Female genital fistula centres all over the country treat about 2000 to 4000 fistula patients.

However, the backlog is severe and it will take about 100 years to treat this condition while ignoring the incidence of about 20,000 per annum). There are 18 fistulae dedicated centres across the country with about 9 centres actively involved with a holistic approach to social reintegration and rehabilitation of women with this condition.

The National Federal Ministry of Health and other stakeholders are dedicated to reducing the incidence of Obstetric fistula by half while devising means of increasing reintegration and social rehabilitation by the same percentage Although, reintegration has been identified to be the weakest link in the management of obstetric fistula, due to low engagement in its services, it has been proven to have components that best treat complications that arise from the condition.

The primary goal of this systematic review is to identify and understand factors that are barriers and facilitators in the reintegration and social rehabilitation of women with complications arising from obstetric fistula. This will serve as an effective tool in the formulation of policy and interventions regarding maternal health and can also help to understand the gap in the treatment of obstetric fistula.
