In 2015, five prospective, randomized trials confirmed the superiority of endovascular thrombectomy over medical management for the treatment of acute ischemic stroke. Recently, a survey of neuro-interventionalists reported the most common first-line mechanical thrombectomy approach to be the direct aspiration first-pass technique.
This approach involves the removal of the clot via an aspiration catheter connected to an external suction source. The availability of effective suction to the middle cerebral arteries and beyond during ADAPT is challenging to ensure, and essential to, the successful removal of the thrombus. While advances in aspiration catheter performance have been associated with faster procedure times, higher cannulation rates, and better outcomes, failure to recanalize with currently available aspiration catheters occurs in approximately one-third of patients.
