Mary is a 20-year-old woman who recently had little baby Sean. Mary has a history of depression but had been managing this well prior to and during her pregnancy. Sean was born prematurely in the Coombe hospital, Mary is parenting alone and no longer in contact with the father. Mary responded and engaged very well within the hospital while the nurses and medical staff helped her manage Sean’s needs prior to discharge. Since discharge Mary has been visited three times by the Public Health Nurse Sarah.
On the last visit, Sarah noticed a deterioration in Mary’s presentation and her interaction with Sean. Mary presented as tired, low on energy, and defensive towards Sarah. While there, Sarah noticed that baby Sean was crying a lot. Mary did not attend to the baby and informed Sarah “he will be grand, he stops eventually”. Mary’s apartment had previously been well maintained and ordered however on this visit Sarah noticed the house was dirty and unkempt and Mary had lost a considerable amount of weight.
On her first two visits, Mary had had a discussion with Sarah about family members who had just left, and there seemed to be a lot of activity around Mary and Sean. On this visit, when Sarah asked how Mary’s family were, Mary became defensive and changed the subject. With careful probing from Sarah, Mary admitted that she is struggling in out of bed in the morning and feels a “crushing sense of sadness every day”.She feels “a huge sense of shame about being a terrible mother” and is afraid of what people will say about her.
